- Consent for Therapy & Evaluation – Permission for us to provide you with therapeutic services and conduct evaluations. By signing this form, you agree to participate in therapy sessions and assessments, and you acknowledge that you understand the purpose and process of these services..
- HIPPA – This release ensures that your health information is handled securely and only shared as you authorize.
- Media Release – From time to time, we may take photographs or film during your visit. These images and videos may be used for promotional purposes, including on our website and social media channels. If you have any concerns or prefer not to be included in these media, please let us know in advance. Your comfort and privacy are important to us.

- Your destination, 2501 Old Vineland Rd, will be on the right. We are just to the right of Bealls.
